Initial Report - Up to 5 hours counselling - Form 1 PD 1 of 2022

A report in this format is to be provided to the Tribunal when the psychologist or psychiatrist has provided up to five hours of counselling to an applicant, and either the applicant is not seeking an award for further counselling, or the counsellor does not recommend more than five hours of counselling.
